Waiver Release Form

WAIVER / RELEASE FORM for CrazYman Training Challenge 2017
 Slade Coaching – Linda Slade – Henry “Skip” Slade

I acknowledge that training for and/or participating in a multi-sport event, (such as triathlon and duathlon), cycling, swimming, running or any other endurance sporting event is an extreme test of my physical and mental limits and that such training and/or participation poses potential risks of serious bodily injury, death, or property damage. I have provided Henry “Skip” Slade and/or Linda Slade (Slade Coaching) with all information which in any way relates to or that could affect my physical health and attest that I am in good health and my physical condition has been verified by a licensed medical doctor.

Furthermore, in return for my participation in this training ‘event’ I, on behalf of myself and my heirs or executors, do hereby:

a) WAIVE, RELEASE, and DISCHARGE Slade Coaching, Henry “Skip” Slade and Linda Slade, directors, administrators, employees, consultants, sponsors, associates, coaches, agents, and affiliates (collectively “Parties”), from any and all lawsuits, awards, liabilities, costs, expenses and disbursements (collectively, “Claims”) for personal injury, illness, death or property damages of any kind which I may have now, or at any time in the future, resulting from participation in this or any other program organized or operated by the Parties;
b) INDEMNITY:  I agree to defend, indemnify and hold harmless any and all Parties from any claims against them related to my training.
c) RELEASE OF CLAIMS: In no event, whether as a result of breach of contract, indemnity, warranty, tort (including any property damage, personal injury, or death, arising from the negligence or gross negligence), strict liability or otherwise shall I assert any Claims against Parties related to my training and racing.
In addition, I acknowledge and agree the opinions, advice, expertise, experience and judgment on injuries, both treatment and prevention, (including diagnosis and prognosis), which I receive from Parties should not be solely considered or followed without guidance from a licensed physician.
List of SOME Possible Hazards
The following is just a PARTIAL list of hazards that are often associated with training for triathlon, duathlon, bike racing, running, swimming, and other such endurance events:
  • Falls resulting in broken bones, sprains, strains, contusions, and equipment damage.
  • Collisions with other athletes or vehicles on the road or trail.
  • Drowning during open water swimming.
  • Saddle sores, crotch rash, neck and back pain, and hand numbness related to bicycle riding.
  • Tendonitis and bursitis, muscle strains and/or tears, and trigger points.
  • Upper respiratory infections associated with heavy training loads.
  • Heart attack and other heart-related symptoms.
  • Gastrointestinal problems such as diarrhea, constipation (due to dehydration issues)
  • Blood in urine or issues trying to urinate, high ketones or protein levels.
  • Men: penile numbness, prostatitis, swollen testicles, and impotence from bicycling; nipple pain from friction
  • Women: nipple pain from friction, vulvar swelling, groin and chest rashes, and yeast infections.
  • Anemia, fatigue, difficulty sleeping, and exposure to pollution, weight loss, immune system issues due to improper recovery and nutritional needs.
  • Post-training depression.
  • Reactions to ocean water quality, possible ear infections, excessive dry skin (worse in cold climates), change in hair color, nasal and sinus infections, and illnesses.
